Effectively managing populations of patients requires new partnerships among providers and payers and the traditional public health community. Participants will learn how to work across sectors to address these drivers, and build new bridges among health care, public health, and community development. Strategies pursued include new uses of data, analytics and information technology; augmenting a non-traditional health care workforce; and employing new delivery and payment models. Participants will learn how leading systems are drawing on these new tools and arrangements to improve the health not just of their own patients, but of overall communities. Nonprofit hospitals and health systems face new obligations under the Affordable Care Act to undertake community health needs assessments and implement community health improvement plans. Participants will learn how the most innovative systems are embracing and executing this new responsibility.
